A number of the services that we are regularly asked to provide are not covered by the NHS and it might be helpful for you to know what these are:
Medical Examination
If undertaken for insurance purposes, Heavy Goods Vehicle Licence, Fitness to Drive, Fitness to undertake certain sports, Pre-Employment etc. Please ask to see our scale of charges.
Medical Reports for Insurance Companies (without examination)
If your insurance company requests such a report it will be based on an examination of your medical record. If you have indicated that you wish to see the report before it is sent, you will need to arrange with the Secretary to call and view it. Alternatively, we can send you your own copy, but will charge you a fee for this service.
Medical Reports for Solicitors (with or without examination)
Your written permission is required and although your solicitor will normally obtain this in advance we as a Practice will request your written permission. Please note that upon receipt of your written consent we will request full payment by the solicitor in advance of either preparing a medico-legal report or undertaking the photocopy of your full medical record.
Providing non-statutory medical certificates
Private medical certificates, holiday cancellation, fitness to travel, passport countersignature, private health insurance etc.
In all the above cases, our scale of charges is consistent with those recommended by the British Medical Association.
Lighter Life – Patient Requests
Lighter life Medicals are not a service provided for under the NHS and would be done by the practice as a Private Service. The lighter life diet plan is not always straight forward and could be dangerous to some patients with existing complex health related conditions.
Please be aware all lighter life medical forms need to be completed by one of the practice doctors and not by our practice nurses.
It is not our intention to be difficult or to prevent you from loosing weight however; it is also not our intention to sign medical forms unless we feel sure it is the right thing to do set against each individual’s clinical health records/medical history and that it falls in line with NICE Guidelines. Should your doctor not feel able to sign your medical form you will need to seek advice from your lighter life consultant as to where you can have this medical completed.
Should you be booked into see a doctor it is advisable that you continue to see the same doctor where possible for follow-up appointments which will help manage further care & future problems should they occur?
If your own doctor feels able to complete your form the privtae fees are set out below.
The cost to provide this service is as follows:
Option 1:
Should a doctor be happy just to sign a brief lighter life form it will cost the patient £17.50.
Option 2:
Should the doctor need to sit and go through your patients notes prior to completing the form it will cost the patient £40.
Option 3:
Should a full medical be required it will cost the patient £120
In order for us to proceed we will need you to leave your forms with the practice secretary who will then speak to your GP and see whether he/she is happy to complete the above and which of the three options is necessary set against your medical records and any clinical conditions you may have.
